Authorised Representative Annual Self-Assessment

This form is provided by AFS Licensed Underwriting Agencies/MGA (Licensee) to their Authorised Representative(s) to capture information from the Authorised Representative to enable the Licensee to conduct an annual review of the Authorised Representative’s compliance with the Authorised Representative Agreement, Financial Services laws and the GI Code of Practice. The Authorised Representative responds to the questions and provides evidence to support their responses to the Licensee.
