Authorised Representative Annual Self-Assessment


This form is provided by AFS Licensed Insurance Brokers (Licensee) to their Authorised Representative(s) to capture information from the Authorised Representative to enable the Licensee to conduct an annual review of the Authorised Representative’s compliance with the Authorised Representative Agreement, Financial Services laws and the Insurance Brokers Code of Practice. The Authorised Representative responds to the questions and provides evidence to support their responses to the Licensee. The form can also be used by the Licensee as a pre-appointment due diligence checklist of each Authorised Representative.
